Funding Request Guide
This Funding Request Guide provides a list of questions you'll be required to answer on our Giving Network's Funding Request Form. To help you prepare, we recommend that you review this Guide prior to completing the Funding Request Form. (Our form will time-out, resulting in an error message, if your computer is idle for 30 minutes.)
In order to complete a funding request for the Giving Network, you will need to provide the following information:

- Name of Organization
- Name of Executive Director of Highest Ranking Staff Member
- Name of Board President/Chair
- Are you a 501(c)3 organization?
- Contact Name
- Title
- Address
- Phone Number
- Email Address
- Describe the mission of your organization.
- Describe the history of your organization.
- How is you organization different from other organizations working on the same issue(s)?
- Year Founded
- Number of Employees
- Number of Volunteers
- Will your organization, or this project, provide volunteer opportunities for Erie Insurance employees, agents or customers?
- Has Erie Insurance been involved with your organization in any way? If yes, please explain the relationship.
- Identify any Erie Insurance employees or agents who serve as volunteers or Board members of your organization.
- Project Name
- Annual budget for your organization
- Project Budget
- Amount Requesting
- Project beginning date
- Project ending date
- Date funds are needed
- Describe your project, including its purpose and detailed information on its goals.
- What specifically are you requesting funding for?
- Describe your audience or population who will be served or impacted.
- How many will directly benefit from this project per year?
- Which of Erie Insurance’s community focus areas does this program support?
- What are the measurable results on this project?
- Briefly describe your plan for evaluating its success.
- List specific media opportunities or ways that Erie Insurance will be recognized.
- List other community partners you will work with to ensure the success of this project. If there are none enter N/A
- Does this program qualify under any tax credit program?
- Budget Worksheet
- Provide details about all other sources of funding you have received.
Additional information is required if requesting $10,000 or more.
- E-mail your annual report to the GivingNetwork@erieinsurance.com or mail your annual report to Erie Insurance Giving Network at Erie Insurance, 100 Erie Insurance Place, Erie, PA 16530.
- Describe your plans to sustain this project in the future.
- Confirm your agreement to send a written report describing the results of the project activities including project evaluation measures to ERIE as requested.
