Overview
ERIE cares about the health of you and your family. That’s why we offer robust health, dental and vision plans as part of our Total Rewards program. Company-paid health care costs reached more than $88.1 million in 2023.
- You pay your share of coverage through payroll deductions.
- Coverage begins your first day of full-time employment or the first day of the plan year (if you enroll).
- Visit any provider but receive greater benefits with in-network providers.
Dependent Coverage
Eligibility
For health, dental and/or vision plans, you may enroll:
- Your legal spouse.
- Your child(ren) who are under the age of 26*, including: a legally adopted child, a child placed with you for adoption, a stepchild, a child for whom coverage is required pursuant to a qualified medical child support order and qualified disabled child(ren) who are age 26 or older who meet certain criteria.
Refer to the eligibility sections of the plans’ Summary Plan Descriptions on ERIEweb for more details.
Be sure to have their Social Security numbers (SSNs) handy, if not already provided. ERIE is required to report SSNs of all enrollees to the federal government. Contact Benefits using the HR Helpline, (814) 870-3747, with questions about dependent eligibility.
Note: ERIE and the plans’ claims administrators treat personally identifiable information such as SSNs as highly confidential and have implemented appropriate controls to protect the privacy and security of this information.
Life Events—Coverage Changes
You must notify Benefits using the HR Helpline, (814) 870-3747, within 31 days of a qualifying life event if you will be adding or removing yourself or eligible dependent(s)* to/from coverage. Life events include birth, adoption, marriage, divorce or loss of eligibility for other coverage.
Dependent Eligibility Verification
When adding new dependents, you will be required to verify their eligibility under the plan rules. Please be sure your dependents meet the eligibility criteria noted above (and detailed in the Summary Plan Descriptions) when making your enrollment decisions. ERIE partners with a third-party vendor that will contact you directly for verification. Gather proof of dependent status, such as birth or adoption certificate and marriage license, for this purpose. You will not be contacted to verify eligibility during 2025 for a dependent who was previously determined to be eligible under ERIE’s plan rules.
*Children will be covered through the last day of the month in which they reach age 26.
